How do I become a Monash IVF patient?
Before you begin treatment with Monash IVF, there are a number of important steps to take.

You will need to ask your doctor for referral for both you and your partner to a Monash IVF Specialist. Under legal requirements specified in the Artificial Reproductive Treatment Bill (ART) 2008,  all patients commencing treatment in Victoria  are required to provide  Police Record Check (PRC) valid from 01/01/2010 as well as signing a consent form for Monash IVF to apply for a Child Protection Order Certificate (CPOC). 

Once your test results are available, your IVF Specialist will meet with you to plan your individualised treatment and will fill out your Treatment Activation Sheet and forward it to the clinic. When these steps are completed your treatment can then commence. A letter will be sent advising that you can commence your treatment.
