Accessing IVF Treatment - Registration

Registering with Monash IVF involves the completion of a registration form

Your IVF Specialist will provide the registration form to you at the time of your consultation. In Queensland this form will be mailed to you prior to your initial consultation with your IVF Specialist.

Registration also provides you with one year's membership to the Monash IVF support group, IVF Friends. IVF Friends is a self-help support group established well over twenty years ago. It is organised by past and present fertility treatment patients and your membership to IVF Friends includes a twelve-month subscription to their monthly newsletter. Please inform reception staff or your patient coordinator if you do not wish to receive the IVF Friends newsletter.

In addition to the monthly newsletter (11 issues per year), the IVF Friends support group offers:
  • Support and information from members
  • Fundraising for IVF research.
Please inform the clinic of any changes of name, address or telephone number. It is vital that we are able to contact you, especially during a treatment cycle and if you have frozen embryos or sperm in storage.

Victorian Registrations

If you register for treatment in Victoria, you must apply for a National Police Record Check and a Child Protection Order Check.
Once all statements are approved by the clinic, treatment can commence.
